SPORTS HALL OF FAME NOMINATION FORM

The St. Thomas Sports Hall of Fame, inspired by John Sage ’66, Jim Michalek ’77 and Jerry Koch ’67, was established by the Alumni Association in 1998 with the intent to commemorate exceptional Eagle athletic accomplishment while celebrating lifelong excellence consistent with the Basilian tradition of teaching goodness, discipline and knowledge. The inaugural class, containing 5 members, was inducted in 2001.

Acclaimed recipients for induction are selected from nominations solicited from all alumni and are reviewed by a special committee of the Association.

A nomination form can be printed and mailed to the St. Thomas High School Alumni Association or you can fill out the online form below.

Based on the following eligibility requirements, please send us your nominations of whom you feel is befitting of this honor:

Graduate, living or deceased, of St. Thomas High School (Exceptions can be made if the person was a coach or had a unique involvement with St. Thomas Athletics.)
Has been a graduate for a minimum of fifteen (15) years
Demonstrates the mission of goodness, discipline, and knowledge in his daily life.
Made significant contributions in athletics while at St. Thomas
Nominations submitted in the past must be resubmitted so that each one is reviewed appropriately by the Selection Committee.
